For your convenience please find below a list of forms for printing out and submitting either at your appointment or by faxing to 07 33857677. If you prefer to fill them out at the Centre prior to your appointment, please arrive 15 minutes before your scheduled appointment time to do so.

Policies and relevant information.
Drugs of Addiction Prescribing Policy – The doctors at Narangba Valley Medical Centre will not prescribe Drugs of Addiction or Schedule 8 Drugs to new patients at their first appointment.
For existing patients requesting Drugs of Addiction or Schedule 8 Drugs a consultation is required with your regular doctor. Scripts will not be done over the phone.
For the safety of our staff and patients there is zero tolerance for violent, rude or
aggressive behaviour at our practice. Anyone who acts in this manner will be asked
to leave and the police may be called. It may also result in the inability to book
further appointments at our practice.
Due to security and privacy reasons , we discourage the use of email to transmit
your private health information to us. Should the need arise emails containing your
personal health information will be encrypted prior to being sent.
